- 1). Contact your employer directly to request a W-2 if you have not received the statement by January 31.
- 2). Call the IRS if your employer is uncooperative or you have not received your W-2 by February 15.
- 3). Provide personal information to the IRS representative including your name, address, phone number and Social Security number, as well as the employer's address and phone number. The IRS will advise the employer to mail the W-2 to you.
- 4). Check the mail for the requested W-2 from your employer or Form 4852 from the IRS. Form 4852 can be used as a substitute for the W-2 if the original statement is not received in time to meet the April 15 filing deadline.
