- 1). Contact your employer. If your W-2 was lost, they should be able to produce a copy. You may have to talk to human resources or the payroll department.
- 2). Call the IRS at 1-800-829-1040 if you have not received the W-2 form by February 16 and cannot contact your employer. You will need to give them the contact information you have for the employer, dates of employment and estimated earnings.
- 3). Use your pay stubs and estimates of earnings to file your taxes by April 15. Use Form 4852.
- 4). File an amendment if you receive the W-2 form after you file your taxes and the information differs from what you filed. You can amend the return by filing a 1040x form.
