How to Obtain a Military Affidavit From the U.S.

    • 1). Go to your local government branch and get the necessary affidavit form. If you need a tax form affidavit, for instance, get that form from the county courthouse or local tax department. If you need a vehicle ownership affidavit form, go to the local Department of Motor Vehicles and get the military affidavit form there.

    • 2). Fill out the form with all requested information. Your name, address, service number, Social Security number and military posting are all necessary.

    • 3). Turn in your form to your commanding officer or to the necessary military department to get it signed and notarized. Not all military affidavit forms have to be signed by your commanding officer; if that isn't necessary, sign the form yourself and give it to the proper authority.
