How to Obtain Immunization Records

    • 1). Visit the hospital or health facility where the immunizations were received. Provide the identity information of the person who was immunized, such as name and date of birth. The health facility will advise you of whether or not the record is available. You may have to pay a small fee to receive a copy of the record.

    • 2). Contact schools that you have attended, including elementary, high school and colleges, to see if your immunization record is still on file. If your record is on file, ask for a copy of it. You may have to submit proof of identity to receive a copy from your school.

    • 3). Contact your local Veterans Administration (VA) if you have served in the military. Provide the VA with the person's name, birthday and Social Security number to receive his military health record. Also advise the VA of your relationship to the person.

    • 4). Check baby books or baby albums. Sometimes items, such as immunization records, are kept in a baby book as memorabilia for the baby or child.
