How to Get Wisconsin Driving Records

    • 1). Go to Wisconsin's Department of Transportation website. Access the Vehicle/Driver Record Information Request Form, entitled form MV2896. Download, print and complete the form.

    • 2). Check the driver licensing fees chart to find out how much you owe for your driver record request. At the time of publication, driver record abstracts cost $7 each, and certified driver records cost $12 each.

    • 3). Mail the complete form and the proper fees to the Wisconsin Department of Transportation, P.O. Box 7995, Madison, WI 53707-7995. The DOT will send your record via the mail.

    • 1). Call 608-261-2566 to reach the Wisconsin Department of Transportation's automated system that processes requests for certified driver records. You can call any day of the week, 24 hours a day.

    • 2). Follow the automated instructions carefully. Enter your date of birth and social security number when prompted.

    • 3). Listen to the message in its entirety. Do not hang up before the automated system explicitly informs you the process is completed, or your request for driving records will not be received.

    • 4). Wait for your bill charging the appropriate fee and driving record abstract to arrive by mail at the address given on your driving record. The bill and driver record will be mailed the next business day.
