How to Apply for Forms to Prove Indian Heritage

    • 1). Obtain a CDIB application. This application is available at the Bureau of Indian Affairs in person or online.

    • 2). Complete the CDIB application by providing your full name, address, date of birth, place of birth, whether or not you were adopted, the names of your birth parents and which tribe your ancestors belonged to. If you know the tribal roll numbers your family is listed on, you need to provide the numbers. In addition to this information you must know the history of your ancestors on both your mother's and father's sides of the family.

    • 3). Provide your father's name, tribe, roll number, date of birth and the date of his death, if applicable. Provide the same information for your father's mother and father, his paternal grandmother and grandfather and his maternal grandmother and grandfather.

    • 4). Enter your mother's full name, date of birth, tribe, roll number and date of death, if applicable. Provide the same for your mother's mother and father as well as your mother's parental grandmother and grandfather and her maternal grandmother and grandfather.

    • 5). Sign and date the application and send it to the tribal offices to which you think you belong. Once your application is received it is investigated. Incomplete applications are sent back to you for completion. If your heritage is proven you are notified by the tribe.
