How to Apply for Certificate of Exemptions at the Pennsylvania DMV

1). Download and print form MV-9 from the Pennsylvania DMV website (see Resources).
2). Complete the form with details about your vehicle, including the VIN, make, model and odometer reading. In section D, you must select a reason from the list for your exemption request. Reasons include the vehicle being an emergency vehicle or an inherited vehicle.
3). Submit the completed form with your "Application for Duplicate Certificate of Title by Owner" form. You can download the title form from the DMV website (see Resources). Send both by mail to:
