How to Change a Last Name in Franklin County, Ohio

1). Go to the Franklin County Probate Court offices and collect an Application For Name Change from the court clerk. Fill out the form with the legal name of the applicant and all of the applicable background information. Submit the application, along with the application fee, to the clerk. The court will then set a hearing date.
2). Within 15 days of submitting the application, publish a change of name notice in a local newspaper with general circulation. As of 2010, the Franklin County Probate Court recommends using The Daily Reporter or The Columbus Dispatch. Make sure to get a receipt from the paper providing proof of publishing.
3). Go to the court on the day of the hearing and testify before a magistrate. Explain why the name change is necessary or desired. Answer any questions the magistrate has and wait for a decision.
