How to Recover a Lost US Savings Bond
- 1). Download the Department of Treasury form 1048 E, Claim for Lost, Stolen or Destroyed United States Savings Bonds. The form can be found on the TreasuryDirect website under "Forms." A link to the form is provided in the Resources below.
- 2). Read through the complete form to determine which portions you need to fill out. There are different sections to fill out depending on the ownership of the bond.
- 3). Complete the applicable portions of the form on your computer. You can type the information directly into the PDF form before printing it out. The areas that can be completed on the computer are highlighted in yellow.
- 4). Print the form with the completed information. Do not sign the form.
- 5). Take the form to your bank or credit union to have the signature(s) certified by a bank official. The form must be signed in the presence of the official and you must show valid identification. The bank will have designated individuals that handle signature certifications.
- 6). Mail the completed form to the U.S. Treasury. The mailing address is:
