About Name Changes After Marriage in Tennessee
- Organization is the key to completing this process thoroughly. Start by making a list of each place you need to change your name. Start a folder to keep track of forms, receipts, proof of mailing receipts and other documents involved in this process.
- The first place to start the name changing process is at the driver's license bureau. To change the name on a Tennessee driver's license, bring your marriage certificate, current driver's license and a fee of $8 to $16 to a Driver's Service Center. A new license will be issued on the spot.
- To change your social security card, new brides will have to fill out a Form SS-5. Bring this form, a marriage certificate and your new state identification to the Social Security office. They will give you a temporary card to use until the permanent one comes in the mail.
- Passports take longer to change. Depending on the age of the passport, brides will need to mail the current passport, a completed Form DS-5504 and the original marriage certificate. Two recent photos and renewal fees may be required. All of these documents should be mailed by a traceable method.
- Once you have a certified marriage certificate, new state ID and social security card, other records may be changed. All records with the maiden name need to be changed. All bank accounts, credit cards, leases, utility bills and voter registration will have to be contacted and provided with the new forms of ID.
