How to Order a Puerto Rico Birth Certificate

    • 1). Download and print out a copy of the birth certificate application posted on the Department of Health's website. Fill out the application and sign it.

    • 2). Make a photocopy of either your driver's license or the information page of your passport.

    • 3). Purchase a postal money order if you do not reside in Puerto Rico. Residents of Puerto Rico can pay with either a postal money order or "Sellos de Rentas Internas" from the Colecturía. Any other forms of payment, such as personal checks and credit cards, are not accepted. As of May 2010, the cost for each certificate is $5, and each additional copy requested on the same application is $4. Make the money order or Sellos de Rentas Internas payable to "Secretario de Hacienda de Puerto Rico."

    • 4). Send the payment, completed application, copy of photo identification and a self-addressed, stamped envelope to:
