How to file for a name change in Kentucky
- 1). There is a form that the state of Kentucky provides that is an application for petition of name change. It is the same form for adults and minor children and you can find it on the Kentucky government website or by visiting your local county clerk.
- 2). Once you get the form, complete it, sign it and notarize it. Most of the Kentucky clerk offices do the notary services for free.
- 3). File the application at the clerk's office with the filing fee. You must be a resident of the state for at least 6 months before filing for a name change.
- 4). At the time of filing, the clerk will schedule a hearing. You must attend the hearing and explain your case to the judge.
- 5). If you are petitioning for a minor child you must make sure that both parents consent or that the non-petitioning parent is notified of the hearing and has the opportunity to object. You will also have to prove to the judge that the change of name would be in the best interest of the child and why.
- 6). The judge will decide if you have a valid plea for a legal name change and if the court will grant the name change.
