How to Change Information on a DD214

    • 1). Correct a military service record by downloading the DD Form 149, Application for Correction of Military Record. Complete the entire form. Specific instructions are outlined on page two of the downloaded form. Attach supporting documents to the form and sign before submission. Mail the completed form and documents to the appropriate branch of the service. Addresses are provided on page two of the form.

    • 2). Correct a discharge status by downloading the DD Form 293, Application for the Review of Discharge or Dismissal from the Armed Forces of the United States. Complete the entire form. Specific instructions are outlined on pages three and four of the downloaded form. Attach supporting documents to the form before submission. The form must be signed before mailing. Mail the completed form and documents to the appropriate branch of the service, as indicated on page two of the form.

    • 3). Reviews of submitted corrections are made by a review board for the branch of the service in which the veteran served. These boards do not make changes to medical records. Sometimes a personal appearance by the veteran or the next of kin, in cases where the veteran is deceased, is required.
