Offical Legal Transcripts
1Provide the official name of the proceeding and the date on the first page of the transcript. If there is more than one volume of the transcript, such as in a trial, label each volume. Identify all persons present on the first page, carrying over to the following pages if necessary. Transcripts must be typed, double-spaced and printed on paper with numbered lines. Each speaker's name and words must begin on a new line. Number each page.

Record a full verbatim written statement of exactly what occurs during the proceeding. Record everything precisely, using official language and correct spelling of technical terms. Record whenever anyone enters or leaves the room, witnesses are sworn to testify, exhibits are marked or handed to the witness, a recess is taken, or proceedings end by noting each activity along with the exact time it occurred.

Identify each speaker by name each and every time he or she speaks. Write the speaker's name followed by a colon and then the exact words stated. Do not correct anything, including grammar, syntax or improper pronunciation. Use technical terms and spell words correctly.

Record the ending time and date of the proceeding on the final page of the transcript. The final page is the court reporter's signature page.
