What USCIS Forms Are Filed With I-485?

    G-1145 -- E-Notification of Application or Petition Acceptance

    • File Form G-1145 if you wish to receive notification by email that USCIS has accepted your application or petition. This notification is sent within 24 hours of acceptance and is merely for informational purposes. The official acceptance notice will follow in the regular mail. The form can be accessed and filled out online, then printed and placed on top of your application. It is not a compulsory form.

    G-325A -- Biographic Information

    • Form G-325A must be completed and included with your I-485 application. It provides the USCIS with the information they need to do a background check, including your full legal name, Social Security number, current and past spouses, current and past addresses in and out of the United States and recent employment history.

    I-693 -- Report of Medical Examination and Vaccination Record

    • You have to file I-693 with Form I-485 unless you have permanently resided in the United States since 1972. Fill out the form and schedule an appointment for a medical examination with a civil surgeon or USCIS-approved medical examiner. Do not sign the form. Take it with you to your appointment together with a recognized form of identity and all your vaccination records. The medical examiner will have you sign the form in his presence and will place it in a sealed envelope together with his findings. Do not open the envelope, but Include it with your application.

    I-864 -- Affidavit of Support Under Section 213 of the Act

    • Most I-485 applications must include Form I-864. This is to be completed by a family member or employer who signs to say that you have adequate means of supporting yourself or she will support you and thereby consent to be your sponsor. This form must be included if you are filing for permanent residence based upon the sponsorship of a family member or if you are gainfully employed in the United States.

    I-485 Supplement A

    • You have to file this form if you entered the United States unlawfully and worked here illegally without a work visa, if you entered the country legally with a work visa but allowed the visa to expire and continued to work illegally, if you entered the United States in transit to another country but stayed here, or if you were a crewman on a ship and worked in the United States without a work permit. The filing fee (or penalty fee) for this form is $1000 and must be included with your application in addition to the regular filing fee.

    Other Forms and Necessary Items

    • All applications must contain two recent color photographs taken with the correct specifications and a photocopy of your immigrant visa in your passport. Different circumstances may require additional forms, which could include I-130 (Petition for Alien Relative) if a parent, sibling or child who is a U.S. citizen or permanent resident is petitioning on your behalf, I-140 (Immigrant Petition for Alien Worker), I-526 (Immigration Petition by Alien Entrepreneur), I-589 (Application for Asylum and Withholding of Removal) and many more.
