How to Apply for a Texas DPS Number
- 1). Contact the Texas DPS by email to request a controlled substances registration application (txdps.state.tx.us). You may also request an application by phone.
- 2). Provide your name, type of medical degree or business, address and phone number in the email. DPS will mail or email the application to you.
- 3). Complete sections "A" through "F" of the application, and sign it in blue ink. Return the application with the $25 application fee to the DPS postal address listed on the form. DPS will not accept faxed copies. Be sure to answer all questions in section "E."
- 4). Wait two to three weeks for a response. DPS must process applications within 60 days.
