How to Get a Copy of My New York State Rap Sheet
- 1). Download a copy of the CHRS application form (see Resources) from the New York State Unified Court System website.
- 2). Fill out the CHRS application form. Provide your name, address, phone number and email address. Provide the first name, middle initial, last name and date of birth for all persons you wish to obtain a statewide criminal record for.
- 3). File the application form by mail or in person and pay the application fee. As of 2011, the fee for obtaining a statewide New York criminal record is $65. If filing by mail, include a self-addressed return envelope with sufficient postage (see Resources).
