How to Obtain Free 1966 Birth Records From California

    • 1). Obtain a copy of the Application for Certified Copy of Birth Certificate via the California Department of Public Health.

    • 2). Fill out the application. This will include your applicant information (such as your name, purpose of request, mailing address, phone number and the names of the people receiving copies). Proceed to the Birth Certificate Information section, where you will fill out the detail on the individual -- such as name, city of birth, county and name of father and mother -- for whom a birth certificate is requested.

    • 3). Sign the sworn statement attached to the application, date it and fill out the location information as well. This states you are authorized to have access to certified copies of these records. If you are not authorized, you will instead receive an informational copy. This will have the same information on it, but it will state "Not a Valid Document" on the paper. Informational copies do not cost. Certified copies will cost $14. Checks needs to be made out to Office of Vital Records. Cash is accepted but checks are preferred.

    • 4). Mail the request to the Office of Vital Records with payment, if necessary. The address is:

      California Office of Vital Records
      MS 5103
      P.O. Box 997410
      Sacramento, CA 95899-7410

    • 5). Wait to receive your return information. This will typically take several weeks. Your return information will be send out by a public notary who works in the Office of Vital Records
