How to Get a Duplicate Medicare Card
- 1). Go to the Social Security Administration website (see Resources) and click on the "Medicare" heading. Click on "Replace Your Lost, Stolen or Damaged Medicare Card." Read the information on the following page and click "Start" at the bottom of the page.
- 2). Enter the requested information, including your Social Security number, name and date of birth. Indicate whether you have moved recently by clicking the "Yes" or "No" button. Click "Continue."
- 3). Verify your identity by entering your place of birth and your mother's maiden name. Click "Continue."
- 4). Verify your address and enter your home phone number, so that the SSA can contact you if necessary. Click the check box if you want proof of Medicare coverage, also called an entitlement letter, sent to you within 10 to 14 days. Click "Submit." Allow four weeks for your replacement card to arrive.
- 1). Call the Social Security Administration at 800-772-1213. Follow the voice prompts to speak with a Medicare customer service representative.
- 2). Tell the representative you need to replace your Medicare card. Give the representative your identifying information, including your name, date of birth, Social Security number, mother's maiden name and address.
- 3). Verify the information you gave and the representative will process your request for you. Allow four weeks for a new card. If you need proof of enrollment sooner than that, you can request an entitlement letter.
- 1). Locate the closest Social Security office to you by calling the SSA, or visiting the SSA website and clicking "Find a Social Security Office" under the "What You Can Do Online" heading.
- 2). Go to the local SSA office and tell the representative you need a replacement Medicare card. Fill out the Medicare replacement card form and give it to a representative.
- 3). Allow four weeks for the card to arrive, or ask the representative to give you an entitlement letter for immediate proof of Medicare coverage.
