How to Change Your Disability From Kentucky to Tennessee

1). Go to the Social Security Administration website at ssa.gov. Click "Disability" at the top of the page. Scroll down the page and click "Change Your Address or Telephone Number."
2). Click "OK" on the Acknowledgement for Change of Address Services."
3). Click "I Agree" on the Public Agreement screen.
4). Enter your demographic information onto the next screen. Enter your name, Social Security number, date of birth and the amount of your disability payment. Once your information is verified, the next screen will allow you to process your change of address.
5). Call 1-800-772-1213 between the hours of 7 a.m. and 7 p.m. Monday through Friday to change your address if you prefer to make the change over the telephone. You can also visit your local Social Security office and submit your address change in person.
